密闭集尿措施在短期留置导尿中的效果及观察

摘要:
目的 对直肠癌术后短期留置导尿的患者采取密闭集尿措施,探讨其对尿路污染的影响。方法 选取2018年3月至2018年12月在华中科技大学同济医学院附属协和医院肿瘤中心治疗并于术后行短期留置导尿的患者60例,随机分为试验组和对照组。对照组集尿袋每日更换;试验组采取密闭集尿护理,包括不随意打开尿袋与尿管之间的连接,不更换尿袋,尽量减少倾倒尿液的次数,排空尿袋时使用独立的且干净的容器,洗手,避免接尿袋开关接触到其他物品等。比较两组患者尿袋内、尿袋与尿管连接处、尿管尖端标本的细菌培养阳性率。结果 对照组尿袋内、尿管与尿袋连接处、尿管尖端的细菌培养阳性例数分别为16例(53.3%)、10例(33.3%)和2例(6.7%),试验组分别为4例(13.3%)、1例(3.3%)和0例(0%)。除在尿管尖端的比较差异无统计学意义外(P>0.05),其他两个部位标本的细菌培养阳性例数比较差异有统计学意义(P<0.05)。结论密闭集尿措施的使用,能有效降低尿路污染的风险;同时能有效降低非必要更换尿袋带来的工作量,进而节约资源,减轻患者的经济压力,值得临床推广。

Abstract:
Objective The aim of this study is to explore the influence of airtight method on the urine collection for the patients with short-term indwelling catheterization after rectal cancer surgery.Methods Sixty patients who underwent short-term indwelling catheterization were randomly divided into the experimental group and the control group. The urine bags were replaced daily by nurses in control group. The airtight method was performed in the experimental group on the basis of the control group including reducing the times of tracts opening, changing of the urine bag, etc. The positive rate of bacterial culture in different part of the urinary tract was compared between the two groups.Results In control group, the incidence of positive bacterial cultures in the urine bag and the junction between the catheter and bag were 53.3% and 33.3%, which were significantly higher than those of experimental group (4[13.3%], 1[3.3%]). There was no significant difference on the number of positive bacterial cultures between the two groups on the tip of the catheter (2[6.7%] vs. 0[0%]) (P<0.05).Conclusions The use of airtight method on the urine collection can effectively reduce the risk of urinary tract pollution and the economic pressure of patients, which is worthy of clinical promotion.
